Indications & Dosage
Prevention of atherothrombotic events in percutaneous coronary intervention (adjunct with aspirin) in patients not already on clopidogrel: Adult: Loading dose 300 mg, to be taken prior to the procedure, alternatively loading dose 600 mg, higher dose may produce a greater and more rapid inhibition of platelet aggregation. Transient or Acute Ischaemic Attack for patients with aspirin hypersensitivity, or those intolerant of aspirin despite the addition of a proton pump inhibitor: Adults: 75 mg daily Prevention of atherothrombotic events in acute coronary syndrome without ST-segment elevation: Adults: 300 mg loading dose followed by 75 mg daily given in combination with 75-100 mg aspirin daily, up to 12 months. Prevention of Atherothrombotic events in acute myocardial infarction with ST-Segment elevation: Adult: 18-75 years: Initially 300 mg, then 75 mg for at least 4 weeks Adult: 76 years and over: 75 mg daily for at least 4 weeks Prevention of Atherothrombotic and thromboembolic events in patients with atrial fibrillation and risk factor for a vascular event (with aspirin) when warfarin is unsuitable: Adult: 75 mg once daily Children: Not recommended.
Contraindications
Severe hepatic impairment, active bleeding such as peptic ulcer or intracranial haemorrhage.
Precautions
Patients at risk of increased bleeding from trauma, surgery, or other pathological conditions, renal impairment, moderate hepatic impairment surgery. Risk of bleeding and blood dyscrasias, per-form blood counts promptly if suspected and when used with other drugs or conditions that affect bleeding. Warn patients to report unusual bleeding. Discontinue 7 days before elective surgery if anti platelet effect not desirable.
Interactions
Anticoagulants, glycoprotein Iib/Iiia inhibitors, thrombolytics, NSAIDs.
Pregnancy
Contraindicated in pregnancy.
Lactation
Contraindicated or not recommended.
Side Effects
Headache, bleeding disorders; including gastro-intestinal and intracranial, diarrhoea, dyspepsia, constipation, decreased platelets, dizziness, duodenal ulcers, eosinophilia, flatulence, gastric ulcer, gastritis, leucopenia, nausea, paraesthesia, pruritus, rash, vomiting. Rare: Vertigo, thrombotic thrombocytopenic purpura.
