Composition
Inj: Ertapenem sodium 1 gm: powder for reconsti-tution.
Indications & Dosage
Abdominal infections, Acute gynaecological infections, Community-acquired pneumonia, Diabetic foot infections of the skin and soft-tissue: BY IM OR BY IV INF: Adult: 1 g once daily. Children 3 months-12 yr: 15 mg/kg twice daily; 13 yrs and above: Same as adult dose. Surgical prophylaxis, colorectal surgery: BY IV INF: Adult: 1 g for 1 dose, dose to be completed within1 hour before surgery.
Administration
IV infusion over 30 min, reconstitute 1 gm vial with 10 ml of WFI, 0.9% NaCI, or bacteriostatic water for injection. Shake well, transfer to 50 ml of 0.9% NaCI. For IM reconstitute 1 gm vial with 3.2 ml of 1% lidocaine HCI injection (without epinephrine). Shake well Use within 1 hour after preparation.
Compatibility
Do not mix with other medications or use diluents containing dextrose.
Stability
Before reconstitution store at a 25 C. Reconstituted IV solution may be stored at room temperature and use within 6 hours or refrigerated, store for up to 24 hours and use within 4 hours after removal from refrigerator. Do not freeze.
Contraindications
Hypersensitivity to beta-lactams, hypersensitivity to amide-type local anaesthesia and in patients with severe shock or heart block.
Precautions
CNS disorders; risk of seizures, elderly, renal impairment; reduce dose to half the regular dose if eGFR less than 30 ml/minute/1.73m2, history of colitis, infant less than 3 months.
Interactions
Probenecid, valproate.
Pregnancy
Avoid unless potential benefit outweighs risk.

Side Effects
Headache, GI disturbance, injection-sitereactions, pruritus, raised platelet count, rash (also with eosinophilia), anorexia, antibiotic associated colitis, asthenia, bradycardia, chest pain, confusion, dizziness, dry mouth, dyspepsia, dyspnea, hypotension, melaena, oedema, petechiae, pharyngeal discomfort, raised glucose, seizures, sleep disturbances, taste disturbances. Rare: Agitation, anxiety, arrhythmia, blood disorders, cholecystitis, cough, depression, dysphagia, electrolyte disturbances, haemorrhage, hypoglycaemia, increase in blood pressure, jaundice, liver disorder, muscle cramp, nasal congestion, neutropenia, pelvic peritonitis, renal impairment, scleral disorder, syncope, thrombocytopenia, tremor, wheezing, dyskinesia, hallucinations
