Composition

Indications & Dosage
Infiltration anaesthesia: BY LOCAL INFILTRATION: Adult: Dose to be given according to patients weight and nature of procedure; max. 200 mg, max. dose 500mg if given in solutions containing adrenaline. Intravenous regional anaesthesia and nerve block: BY REGIONAL ADMINISTRATION: Adult: See literature. Pain relief (in anal fissures, haemorrhoids, pruritus ani, pruritus vulvae, herpes zoster, or herpes labialis), Lubricant in cystoscopy, Lubricant in proctoscopy: TOPICAL APPLICATION: Adult: Apply topical Jelly/Oint/Cream/Solution 1-2 ml as required, avoid long-term use Sore nipples from breast-feeding: TOPICAL APPLICATION: Adult: Apply topical Jelly/Oint/Cream/Solution 1-2 ml using gauze and wash off immediately before next feed. Anaesthesia before venous cannulation or venipuncture: TOPICAL APPLICATION: Adult & Children above 1 yr: Apply 1-2.5 g cream, apply thick layer to small area(2.5 cm62.5 cm) of non-irritated skin at least30 minutes before procedure; may be applied under an occlusive dressing; max application time 5 hours, remove cream with gauze and perform procedure after approximately 5 minutes. Children 1-2 months: Apply up to 1 g cream, apply thick layer to small area (2.5 cm62.5 cm) of non-irritated skin at least 30 minutes before procedure; may be applied under an occlusive dressing; max. application time 60 minutes, remove cream with gauze and perform procedure after approximately 5 minutes; 3-11 months: Apply up to 1 g, apply thick layer to small area (2.5 cm62.5 cm) of non-irritated skin at least 30 minutes before procedure; may be applied under an occlusive dressing; max application time4 hours, remove cream with gauze and perform procedure after approximately 5 minutes. Postherpetic neuralgia: TOPICAL APPLICATION: Adult: Apply plaster once daily for up to 12 hours, followed by a 12-hour plaster-free period; discontinue if noresponse after 4 weeks, to be applied to intact, dry,non-hairy, non-irritated skin, up to 3 plasters may beused to cover large areas; plasters may be cut. During delivery in obstetrics: TOPICAL APPLICATION: Adult: spray up to 20 doses.
Administration
Administer IM injection in deltoid muscle only. IV injection administered by direct IV injection and continuous IV infusion. Product containing 40, 100, or 200 mg/ml should not be administered by direct IV injection without prior dilution. Usually 1 or 2 gm of Lignocaine is added to 1000 ml of dextrose 5% to form a 1 or 2 mg/ml (0.1 or 0.2 %) solution.
Compatibility
Dextrose 5%, or with NaCI 0.45%, NaCI 0.9%.
Stability
Stored at room temperature and protect from heat and freezing.
Contraindications
When used by regional administration: Atrioventricular block, application to the middle ear (risk of ototoxicity), avoid injection into infected or inflamed tissues, preparationscontaining preservatives should not be used for caudal,epidural, or spinal block, or for intravenous regional anaesthesia (Biers block), severe myocardial depression Topical application: Should not be applied to damaged skin, sino-atrialdisorders
Precautions
When used by regional administration:Congestive cardiac failure, debilitated patients, elderly, epilepsy, hypovolaemia, impaired cardiac conduction, impaired respiratory function,myasthenia gravis, post cardiac surgery, shock, acute porphyria, renal and hepatic impairment. To avoid excessive dosage inobese patientsdose should be calculated on the basis ofideal body-weight.
Interactions
Propranolol, cimetidine, antiarrhythmics, diuretics.
