CARDIAC THERAPY Phosphodiesterase type-3 inhibitor ATCO

Composition

Inj: Per ml: Milrinone 1 mg; 10 ml (10 mg) amp.

Indications & Dosage

Short-term treatment of severe congestive heart failure unresponsive to conventional maintenance therapy (not immediately after myocardial infarction), acute heart failure, low output states following heart surgery: BY IV INJ: Adult: Initially 50 mcg/kg, given over 10 minutes, followed by (by intravenous INF) 375-750 nanograms/kg/min usually given following surgery for up to 12 hours or in congestive heart failure for 48-72 hours; max. 1.13 mg/kg per day.

Administration

IV: Infuse via infusion pump continuous IV infusion 20 mg per 100 ml 0.9% NS or D5W.

Contraindications

Severe hypovolaemia, acute myocardial infarction.

Precautions

Heart failure associated with hypertrophic cardiomyopathy, stenotic or obstructive valvular disease or other outlet obstruction, renal dysfunction; reduce dose and monitor response if eGFR less than 50 ml/minute/1.73m2 consult product literature for details. Monitor blood pressure, heart rate, ECG, central venous pressure, fluid and electrolyte status, renal function, platelet count and hepatic enzymes. Correct hypokalaemia. Monitor fluid electrolytes balance.

Side Effects

Ectopic beats, headache, hypotension, supraventricular arrhythmias, ventricular tachycardia, chest pain, hypok alaemia, thrombocytopenia, tremor, ventricular fibrillation. Rare: Anaphylaxis, bronchospasm, rash
