Composition

Inj: Sodium bicarbonate 8.4% (84 mg/ ml equal to 10mEq/10 ml); 25 ml vial

Indications & Dosage

Cardiac arrest: Infants: 1 mEq/kg slow IVP initially; may repeat with 0.5 mEq/kg in 10 min one time, or as indicted by patients acid-base status. Children & Adults: 1 mEq/kg IVP initially; may repeat with 0.5 mEq/kg in 10 min one time or as indicted by patients acid-base status. Metabolic acidosis: If blood gases & pH measurements are available: Neonates, infants & children: HCO3 (mEq) = 0.3 x weight (kg) x base deficit (mEq/L) or HCO3 (mEq) = 0.5 x weight (kg) x [24 - serum HCO3 (mEq)]. Adults: HCO3 (mEq) = 0.2 x weight (kg) x base deficit (mEq/L) or HCO3 (mEq) = 0.5 x weight (kg) x [24 - serum HCO3 (mEq)]. If acid-base status is not available: Dose for older children & Adults: 2-5 mEq/kg IV INF over 4-8 hr; subsequent doses should be based on patients acid-base status.

Administration

In neonates &infants, use the 0.5 mEq/ ml solution or dilute the 1 mEq/ml solution 1:1 with sterile Water for Injection. In children & adults, the 1 mEq/ ml solution may be used Administer slowly (maximum rate in neonates & infants: 10 mEq/min); for infusion, dilute to a maximum concentration of 0.5 mEq/ ml in dextrose solution and infuse over 2 hours (maximum rate of administration: 1 mEq/kg/hour).
