Indications & Dosage
To reduce proteinuria and slow kidney function decline in adults with Primary Immunoglobulin A (IgA) Nephropathy (IgAN) who are at risk for disease progression: Adults: Initiate treatment with 200 mg orally once daily. After 14 days, increase to 400 mg once daily, prior to the morning or evening meal as tolerated. When resuming treatment after an interruption, consider re-titration.
Contraindications
Concomitant use with angiotensin receptor blockers (ARBs), ERAs, or aliskiren. Pregnancy
Precautions
Hypotension, acute kidney injury; monitor kidney function periodically, risk of hepatotoxicity; monitor ALT/AST levels weekly; dose adjustment or an interruption is required, hyperkalemia; monitor serum potassium periodically, fluid retention. Monitor ALT/AST levels weekly; dose adjustment or an interruption is required.
Interactions
CYP3A inhibitors, antacids, acid reducing agents, non-steroidal anti-inflammatory drugs (NSAIDs), selective cyclooxygenase (COX-2) inhibitors, CYP2B6, 2C9, and 2C19 substrates, sensitive P-gp and BCRP substrates, agents increasing serum potassium.
Pregnancy
Contraindicated in pregnancy.
Lactation
Not recommended.
Side Effects
Hepatotoxicity, embryo-fetal toxicity, hypotension, acute kidney injury, hyperkalemia, fluid retention, hyperkalemia, hypotension (including orthostatic hypotension), peripheral edema, dizziness, anemia, acute kidney injury, transaminase elevations.
