UNITINASE

Composition

Inj: Streptokinase 1.5 MIU per vial (powder) Inj: Per vial: Streptokinase 100000 IU/250000 IU/600000 IU/750000 IU powder in vials.

Indications & Dosage

Acute myocardial infarction: Adult: 1 500 000 units, dose to be given over 60 minutes,within 12 hours of onset of symptom. Deep-vein thrombosis, Pulmonary embolism, Acute arterial thromboembolism, Central retinal venous or arterial thrombosis: Adult: 250 000 units, dose to be given over 30 minutes, then 100 000 units every 1 hour for up to 12-72 hours, according to condition with monitoring of clotting parameters (consult product literature) Acute pulmonary embolism: Adult: 3 million unit dose over 24 hr.

Administration

For IV infusion, give continuously or intermittently;reconstitute with sodium chloride 0.9%, then dilutefurther with Glucose 5% or Sodium Chloride 0.9% after reconstitution. Do not administer by IM injection, for IV or infra-coronary use only. Infusion pump is required. Use in-line filter >0.8 micron. CANNULA OCCLUSION: 250000 units into cannula, clamp for 2 hours then aspirate contents and flushwith NS.

Compatibility

Stable in D5W, NS. Incompatible with dextrans

Stability

Intact vials should be stored at room temperature stability ofparenteral admixture at roomtemperature (25C) is 8 hours and atrefrigeration (4C) is 24 hours. Reconstituted solutions should berefrigerated.

Contraindications

Allergy and cross-sensitivity; Contra-indicated if previous allergic reaction to streptokinase, recent streptokinasetherapy (5 days streptokinasepreviously), severe liver or kidney damage, recent haemorrhage, haemorrhagic diathesis, severe hypotension, streptococcal infections, sub-acute bacterial endocarditis, recent surgery, bleeding tendencies.

Precautions

Obtain haemostatic status of patient before starting therapy. Followup with anticoagulant therapy. Seeliterature.

Interactions

Anticoagulants, drugs affecting platelet function.

Pregnancy

Contraindicated in pregnancy.

Lactation

Side Effects

Hypotension, arrhythmias, Febrile reactions. Rare; Guillain-Barrsyndrome, anaphylaxis Haemorrhage.
